REGISTRATION INFORMATION

REGISTRATION INFORMATION
Why wait? Register early for programs to ensure you get a spot. Most programs require registration at least two weeks before the scheduled session begins. Be sure to check the class or activity you’re interested in for registration dates, times, and locations.
 
The Summer 2008 sessions begin Monday, June 9, 2008 and end Friday, August 8, 2008.
 
There are three ways you can register:
 
1.      ON SITE (check / cash/ money order)
      You can stop into to the center where your class or program will be offered and register and pay on site. Payments can be made at the centers, and we accept checks, money orders, and cash. If you’re registering for RiverbendArtCenter, Riverbend will accept credit card payments onsite.
 
2.      MAIL-IN (check / credit card / money order )
      You can download a registration form from the web site, print it, fill it out and mail it in with your payment (please, do not mail cash). Please make checks or money orders payable to the City of Dayton. Mail form and payment information to:

3.      FAX (credit card / household balance only)
      You can download a registration form from the web site, print it, fill it out and fax it in with a credit card payment. Fax form with payment information to:

Full payment for classes must be made at the time of registration. Resident fees are for those participants who either live or work within the Dayton city limits. Generally, program fees for non-Dayton residents will be an additional 25 percent. Fees will be refunded if a class is full.
 
PAYMENT METHODS
The City will accept cash, personal checks, credit cards (Visa, MasterCard and Discover) and money orders for the payment of fees. Please note which of these forms of payment is acceptable for the method of registration you choose. The City cannot accept any debit cards for payment. The City will charge a $25 service fee for all checks not honored for any reason by the bank. Checks should be made payable to: City of Dayton.
 
 
CONFIRMATION
Once we have processed your registration, we’ll mail you a confirmation showing which classes you are enrolled. If we were unable to place you in a class, the fee for that class will be placed in your family’s household account as an account credit, and this, too, will be noted on your confirmation. You may:
 
• use the credit for future courses; or
• request a full refund. All refunds will be paid by City check; please allow three weeks for processing.

RESIDENT/NON-RESIDENT POLICY
People who live within the Dayton city limits or work within the Dayton city limits pay Dayton taxes. These taxes help support the facilities and programs offered through the Department of Recreation and Youth Services. As such, those who pay taxes in our City are considered City “residents” or “workers” for registration and fee purposes. Those living or working outside of the City’s corporate limits are considered “non-residents/workers”. Generally, program fees for non-Dayton residents/workers will be an additional 25 percent. Not all programs or activities are resident/non-resident fee based.
 
To verify your residency status, you will need to provide adequate validation in order to receive the resident rates. To do so, please provide a copy of one of the following when registering by mail, on site or by fax:
 
For those living in the Corporate Limits of Dayton
  1. Valid Dayton recreation resident card
  2. A valid Ohio driver’s license with Dayton address
  3. Current utility bill with Dayton address, accompanied by a valid photo I.D.
  4. Checkbook with Dayton address, accompanied by a valid photo I.D.
 
For those working in the Corporate Limits of Dayton.
  1. A pay stub showing the name and location of employer with a driver’s license
 
When registering in person, you’ll need to show staff one of these items. Failure to provide adequate proof of residency will result in you being charged the non-resident rate, and your family will be processed as non-residents.
 
REFUND POLICY
All refunds must be requested through a Department of Recreation and Youth Services refund form. All forms must be completely filled out, will be subject to the Department refund policy as outlined below and approved by the Department Director.
 
Cancelled Classes
A full refund will be issued in the name of whoever paid the registration if the cancellation is initiated by the Department of Recreation and Youth Services due to insufficient enrollment.
 
Before the program begins
Refunds will be made only before the start of the program for the following circumstances:
 
  1. When the refund is requested 5 business days before the first class provide that it does not reduce the participation level below the required minimum.
  2. When the participant becomes ill and presents a doctors statement before the first class.
 
After the program begins
Refunds will be granted up to the beginning of the second week of classes. These refunds will be prorated and subjected to a $10.00 administration fee. All refunds will be issued by City check and may take two to four weeks to process. Refunds will only be issued to the person that paid the fee. No refunds will be issued after the beginning of the second week of the class.
 
Refunds due to illness
Refunds will only be issued if the participant becomes ill and is no longer able to participate in the remainder of the program. A doctor’s statement must be provided. All refunds issued will be prorated from the date of notification to the Department of Recreation and Youth Services. 
 
Trips- Since trips require reservations weeks ahead of time, the Department of Recreation and Youth Services cannot process refunds for persons requesting a refund (no exceptions).
 
Class transfers- Persons wishing to transfer from one class to another may do so, providing that the transfer does not reduce the participation level below the required minimum.
 
Memberships/Passes- Memberships and passes are non-refundable and non-transferable
